Emergency services were alerted to a single-vehicle crash on State Highway 30 near Matahi Road shortly before midnight last night.
Hato Hone St John said six patients were assessed at the scene.
Two were transported to Rotorua Hospital in a serious condition and two transported to Whakatane Hospital with moderate injuries.
It said four ambulances and one rapid response vehicle responded to the scene.
Police said it is making enquiries into the crash.
