Police said the fatal crash has left the road closed since around 7am this morning.
Police said one person was found deceased at the two vehicle crash on State Highway 2/Matawai Road, Waipaoa.
Another person received serious injuries and was transported to hospital by ambulance.
Police advise the road will remain closed while the Serious Crash Unit conduct a scene examination.
Enquiries into the circumstances of the crash are ongoing.
Hato Hone St John was notified of the crash at 6:49 this morning.
One ambulance and one rapid response vehicle responded to the scene.
One patient was transported to Gisborne Hospital in a serious condition.
Following the crash, Police said motorists using State Highway 2 are advised there will be "extensive delays, of several hours, as a result of the State Highway 2/Matawai Road crash at Waipaoa."
"The diversion is also extensive and motorists, including those in commercial vehicles, are asked to delay travel or consider alternative options."
NZTA said the road is closed between Te Karaka and Lavenham Road and will remain closed until further notice.
